Introdução. Caracterizamos mudanças nos hábitos alimentares de estudantes de Medicina e seu possível impacto na saúde e no desempenho académico. Objetivo. Descrever padrões antes e durante a vida universitária e identificar diferenças por sexo e responsável pela preparação dos alimentos. Metodologia. Estudo transversal, descritivo e quantitativo em Manizales, Colômbia (n=106; amostragem intencional). Questionário digital sobre preparação de alimentos, padrão alimentar, pequeno-almoço, consumo de UPF, frutas/legumes, hidratação e energéticos. Análise em Jamovi 2.6.44: descritiva; Wilcoxon para comparações emparelhadas; Kruskal–Wallis e post hoc Dwass–Steel–Critchlow–Fligner; p<0,05. Resultados. Houve uma diminuição nas refeições diárias (3,05 para 2,60; Wilcoxon: W=1531, p<0,001; r=0,52). A omissão de refeições foi frequente (59,4%), principalmente por falta de tempo. Apenas 31,1% tomam café da manhã diariamente; 10,4% quase nunca. 8,5% consomem ≥5 porções/dia de frutas/vegetais. A ingestão de água foi de 3,34 copos/dia (mediana=3) e o consumo de energéticos mostrou alta assimetria (média=1,41/semana; mediana=0). Percepções: alimentação menos saudável (64,1%), horários irregulares (70,8%) e menor atividade física (71,7%). Por sexo, maior omissão nas mulheres (χ²(1)=4,04; p=0,044; ε²=0,038). Conclusões. A transição para a universidade está associada a padrões menos saudáveis. São urgentes políticas no campus que facilitem uma alimentação equilibrada, hidratação e educação nutricional para manter a saúde e o desempenho académico.
