Testicular cancer is an entity that affects men relatively frequently; it is mostly represen-ted by germ cells. In this study, two cases are presented where there is an important and unusual giant locoregional progression. Associated symptoms include the appea-rance of a testicular mass with or without pain, increased scrotal volume, edema, and its progression is subject to timely diagnosis and treatment. The diagnosis of this pathology includes a clinical approach, anamnesis and detailed clinical examination that is complemented by imaging studies, tumor markers, as well as the definitive histopathological study and, subsequently, complete oncological staging. The treatment includes a multidisciplinary approach that integrates surgical intervention for orchiectomy; Depending on the staging, it may require retroperitoneal lymphade-nectomy and, additionally, adjuvant chemotherapy, achieving significant cure rates. After the detailed analysis of the cases presented and the review of the literature, it was evident that adequate management and timely follow-up lead to acceptable therapeutic responses; The importance of adequate adherence to multidisciplinary oncological treatment is then highlighted as a key to the outcome and complications derived from this entity.
