A little more than a year after the first descriptions of the new coronavirus disease 2019 (COVID-19) and to date (30-01-2021), after 102.403.247 reported cases and more than a million deaths worldwide, it continues to be challenging to approach and manage of serious cases, especially that of the most vulnerable population: the elderly. We present a case of a previously functional 83-year-old man with unmanaged comorbidities, with shortness of breath that rapidly evolved to respiratory failure, who was diagnosed with SARS-CoV-2 infection. A COVID-19 infection approach in the elderly is made here, based on the algorithms used in Colombia for diagnosis and management, finally we present its complications and sequelae that have been reported more frequently in the elderly, as well as the strategies for its prevention and early treatment.
