Luego de un poco más de un año de las primeras descripciones de la nueva enfermedad por Coronavirus 2019 (COVID-19) y hasta la fecha (30-01-2021) tras 102.403.247 casos reportados y más de dos millones de muertes a nivel mundial, sigue siendo un reto el abordaje y manejo de los casos graves, en especial el de la población más vulnerable: los ancianos. Se presenta un caso de un hombre 83 años previamente funcional y comorbilidades sin manejo, con cuadro clínico que evolucionó rápidamente a falla respiratoria, a quien se identifica infección por SARS-CoV-2. Se hace el enfoque de infección por COVID-19 en ancianos, con base a los algoritmos utilizados en Colombia para diagnóstico y manejo, presentándose finalmente sus complicaciones y secuelas que se han referido con mayor frecuencia en esta población, así como estrategias para su prevención y manejo temprano.
