Introduction: Agenesis of the basilar artery is a variation of the cerebral arterial cir-cle or the circle of Willis, which is not very common in the literature, and the reported cases are scarce and are found in association with other cerebrovascular anomalies. It is important to highlight that in less than 40% of the cases, the circle of Willis is undamaged and well-developed; the rest of the cases present different anatomical variations, which means that these are not entirely uncommon; however, abnormalities of the posterior arterial circle and basilar artery hypoplasia are rare and have not been extensively described. In this paper, we expose a variation of the circle of Willis, of the posterior circulation, specifically pure basilar agenesis, an interesting case report that adds to the description of this type of variation of the arterial circle, which provides little information for its study and diagnosis. Objectives: We present a case report on pure agenesis of the basilar artery to contribute to and enrich the literature on this topic. Clinical case: A 45-year-old female patient, with no significant history presented with a predominantly frontal headache with warning signs. Simple skull tomography was requested, and a lesion suggestive of an aneurysm was observed in the intrapeduncular system. Diagnostic arteriography was then performed where an aneurysm was ruled out, but an important finding was the absence of the basilar artery with a predominance of the vertebral arteries. Patients who do not require additional interventions will be discharged for outpatient check-ups. Conclusion: Basilar agenesis is a variation of the Circle of Willis of the posterior sector; rare, there are no exact data or figures in the literature on the matter; therefore, we consider that with our case report, we provide information, and we encourage it to be carried out. Diagnosis was suspected, and the data were published in this regard.
