Warfarin use is associated with appearance of Adverse Drug Reactions like overanticoagulation, this generates in the elderly more morbidity, hospitalized management and increased bleeding risk through associations with another illnesses and drug and food interactions. This anticoagulant is a coumarin which acts through inhibition of extrinsic coagulation pathway factors, because of it’s effect on Vitamin K cycle, and it’s follow with the International Normalized Ratio (INR) finding more hemorrhage probability in values above 4. The approach and management of elevated INR must be founded in patient characteristics and bleeding category in order to decide necessity of Vitamin K, Fresh Frozen Plasma or Prothrombin Complex Concentrate and thus allowing safe return to therapeutic anticoagulation levels.
